Sample Visa Application

Start by obtaining a visa application from your consulate’s website. Applications may differ from consulate to consulate, so some questions may differ slightly from those that are shown below. If you are unable to determine the answer, please contact the KU Consortium Office at 785-864-7576. Be sure to print your answers very clearly. If your handwriting is considered illegible, your application could be denied. Complete all portions of the visa application. If a particular question does not apply to you, you must write NA. Most questions are self-explanatory, but the following points may prove helpful.

2. Surnames at birth: Write NA unless your last name has changed for any reason (i.e. marriage, adoption, etc.)

3. First names: Example: Christopher

4. Date of birth: Example: September 25, 1987

5. ID-number (optional): NA

6. Place and country of birth: Example: Kansas, USA

7. Current nationality/ies: Example: American (if you do not hold a U.S. passport, additional paperwork may be required for your visa. Please inquire of the Consulate).

8. Original Nationality: Example: American

9. Sex: Example: Male

10. Marital status: Example: Single

11. Father’s name: Example: Thomas Columbus

12. Mother’s name: Example: Jane Columbus

13. Type of passport: National Passport

14. Passport number: Passport number printed on the title page of valid passport

15. Issued by: Printed on title page of valid passport Example: US Department of State: National Passport Agency

17. Valid until: Example 04 April 2015

18. If you reside in a country other than your country of origin, have you permission to return to that country? For Non- US Passport Holders, be sure to mark this answer clearly and give details of your visa status. If you are a US Passport Holder mark NA.
